Struggling with under eye circles? You may be suffering from loss of fat under the eyes causing a hollowed look. If you are fair skinned, or have thinning skin under the eyes (very common as we get older), those under eye veins may be adding to the dark hues we can see under the eyes.

Assessment is critical:

Proper examination and assessment should be performed to look for eye conditions such as significant lower lid fat prolapse, dermatochalasis, any degree of periorbital hollowing, and assessment of under eye skin texture and pigmentation. Some patients may need botox with filler to soften fine lines, or laser resurfacing for loose skin prior to fillers.

A very important aspect of aesthetic fillers is to manage your expectations. Some of our patients are very savvy and occasionally will ask for a particular filler by name. Some opt for a more permanent treatment and forego filler for surgery. If realistic expectations can be met, then you may be a great candidate for aesthetic fillers.

Here comes the quick solution!

Hylauronic Acid is a gentle type of filler that’s made from sugar and naturally occurs in the body’s skin and cartilage. Topical or local anesthesia is applied prior to a series of injections placed under the eyes that allows the filler to simulate the appearance of thicker fat pads under your skin. It’s a 30 minute procedure and can cost between $600 – $1200. Depending upon one’s facial structure and rate of metablosim, the filler can last up to 9 months to a year.

Injector knowledge and experience is critical:

Not all Hylauronic Acid fillers are alike. After some research, I have found that Bolotero Balance, Restylane and Juvederm Ultra are all great choices for under eye dermal fillers. Bolotero is best suited for those with thinning under eye skin as its special properties do not cause what is sometimes described as ‘that bluish hue’ seen under thin skin also known as the Tindle Effect. For darker skinned individuals, Restylane or Juvederm is an ideal choice which may even last longer than Bolotero.

Therefore, it is extremely important that your injector understands the significant differences among the available FDA approved products and their appropriate areas of use and the proper technique of application.

What to know prior to lower lid rejuvenation:

Avoid motrin, aspirin, fish oil, alcohol several days prior to treatment. After a relatively painless procedure, all you need is ice.  Avoid high intensity activities or sleeping face down for 24-48 hours. Plan a return follow-up appointment in 1 week after your treatment.

Complications and side effects:

Common side effects include bruising, swelling, and temporary discomfort at the injection sites. Some fillers are known to attract water and some edema is considered normal. There are a few patients that are more susceptible to edema and in rare cases can develop chronic edema. Hylanex is an enzyme that is used to dissolve fillers made of Hylauronic Acid in certain instances where filler needs to be dissolved.
