At the Slate, we pride ourselves on offering the best care for our clients. Please make yourself aware of our practice policies and procedures described below. Our policies have been designed to ensure you enjoy a truly peaceful and relaxing visit with us.

CONSULTATION  Slate is dedicated to helping you achieve your aesthetic goals. We offer complimentary consultations to better understand your needs. Based on a private analysis and conversation, we partner with you to design a treatment plan that will best meet your expectations.

SCHEDULING  Please call ahead and we will schedule you for a consultation or treatment. For your convenience, we recommend scheduling your next treatment before leaving our facility. Due to the customized nature of many of our treatments, appointments can sometimes exceed their scheduled time. Please be patient and know that you will also receive the same exceptional service and personal attention.

CANCELLATION POLICY  A scheduled appointment at the Slate reserves the time of our professionals. Out of respect for our staff’s time, we do require a credit card to secure all appointments. Cancellations are accepted up to 24 hours prior to your appointment without incurring a charge. Cancellations with less than 24 hours or no-shows require a $100 service fee which will be charged to your credit card. Thank you for your consideration.

ARRIVAL  Please arrive 15 minutes prior to your appointment, so you’ll have plenty of time to unwind, and give us the 411 on what’s new in your life (aka paperwork). As we are a medical spa, we require that all new clients fill out a medical/skincare history and treatment consent forms. Paperwork and consent forms must be updated annually. There’s nothing fashionable about being late, you’ll just deprive yourself of the full treatment time.

PHOTOGRAPHY  Treatments include pre and post treatment photography. All photographs are private and covered by HIPPA laws. Post treatment and all follow-up visits do include photography.

PAYMENT  We accept cash, Visa, Mastercard, American Express, Discover, Care Credit & Financing Options.  All treatments/products must be paid for at or prior to the time of treatment/purchase. There are no discounts when using Care Credit or Financing. We accept personal checks as a form of payment for treatments only, at least 5 days prior to treatment appointment, however, if a check is returned for insufficient funds, a fee of $30 will be added to the amount due.

RETURN POLICY – Medspa ServicesThere are no refunds for the services performed by the staff of Slate, however, remaining treatments can be used as credit towards any service at Slate. There are risks and possible complications for every treatment procedure. Every individual is unique. Results may vary. Although the vast majority of patients achieve desired results, there is no guarantee of results for any treatment. | Retail ProductsRetail Products are eligible for a refund if you have had an adverse skin reaction. Returned items are subject to inspection by Slate staff. All original product packaging and its contents must be returned within 7 days. Returned products may not be altered, over 10% used or damaged. We reserve the right to deny multiple refunds on products purchased and product types returned more than once. Gift certificates are not refundable or transferrable.

FOR YOUR ENJOYMENT  For their safety, and for the courtesy of other guests receiving relaxing services, children under 13 years of age are not permitted. Please put cell phones on silent. This is your time to relax and be in the moment. Inform our front desk staff if you have any special needs or concerns to allow us to ensure your time spent with us is a memorable experience.

VALUABLES  We are not responsible for lost or stolen items and encourage you to leave valuables at home.

For the comfort of our guests, the Slate is a Non-Smoking facility.

We wouldn’t be the trendsetters that we are if we weren’t constantly expanding our services. Although the Slate makes every effort to keep its advertising material updated, please note that prices and services are subject to change at any time. We love having you as a client, but we reserve the right to refuse service at any time, to anyone, for any reason.
